Quality improvement programs: Consistent execution drives year-end performance dollars

EQUIPP can serve as a practical roadmap for improving pharmacy performance through data-driven insights and simple, repeatable routines. Pharmacies that perform well do not treat quality improvement as a “check-the-box” activity. Instead, they build it into daily workflow, focus on the highest-value opportunities, and maintain a steady rhythm throughout the year. 


Best-Practice Steps to Help Earn Performance Dollars
 

To support stronger results, pharmacies should consider the following best practices: 

  • Assign an owner or owners for EQUIPP performance tracking and patient review. 
  • Review patient outliers weekly as they refresh in EQUIPP. 
  • Focus time where it matters most by prioritizing the programs and measures with the greatest payout opportunity.
  • Segment patients into actionable groups, such as: 
  • Patients near miss or in jeopardy of falling below 80% PDC 
  • Patients who may need additional support, including those with recent discharges, transitions of care, cost barriers, or frequent abandoned prescriptions 
  • Patients not currently on a preferred formulary medication 
  • Share key patient details with the team and/or add notes in the pharmacy system so everyone understands the next best action. 
  • Connect high-value patients to adherence solutions using the appropriate touchpoints, such as calls, texts, medication synchronization, delivery, or packaging. 
  • Monitor month-over-month performance and share progress with the pharmacy team.
  • Stay consistent throughout the year because steady, ongoing work is what drives stronger year-end outcomes. 

 

Review Program Requirements in EQUIPP 

To understand what is needed to qualify for performance dollars, pharmacies should review program requirements directly in EQUIPP: 

  1. Go to the My Programs tab in EQUIPP. 
  2. Click Program Info in the top right corner.
  3. Review the details, thresholds, and requirements needed to qualify for performance dollars. 

Strong Quality Improvement Program results come from starting early, staying consistent, and making sure someone owns the work. By reviewing outliers weekly, focusing on the most meaningful opportunities, and following program requirements in EQUIPP, pharmacies can make steady progress toward payer thresholds, reduce the stress of last-minute gap closing, and help maximize year-end pay-for-performance opportunities.
